This is the biggest part of the range and the mechanism the brand is known for. A ratchet belt has no holes: the strap runs through a buckle that grips a track of fine teeth on the underside, and the lever on the side releases it.
The practical difference is the size of one step. A holed belt moves in inches. A ratchet belt moves in fractions of an inch, and that is the whole reason it sits the same after lunch as it did at breakfast. The trade is that the strap has to be cut to your waist once, with scissors, before the first wearing.

Ratchet Belts: what people ask
Do I really cut the belt?
Yes, once. The buckle comes off, you trim the plain end to your waist, and the buckle goes back on. Every set in this department carries the waist range it is cut for, so the amount you remove is small.
What if I cut too short?
There is no way back, so the honest advice is to try it long first. Fasten the belt uncut, mark where the buckle sits, and take off less than you think. The strap can always be shortened again.
Does the buckle wear out before the strap?
It is the other way round for most people. The track is metal and the strap is leather or webbing, so the strap creases at the fold long before the teeth stop gripping. Straps are sold on their own for that reason.
